Case Discussion The natural history of abdominal aortic aneurysms (AAA) is that of slow expansion and rupture with devastating consequences. The risk of rupture is proportional to the size of the aneurysm and the rate of growth. An aneurysm is defined as a 50% enlargement of a vessel. The normal infrarenal aorta measures 2 cm on average in men and 1.8 cm in women. The normal common iliac artery measures approx 1.2 cm in men. Most AAA are fusiform as opposed to saccular which are slightly more frequent in thoracic aortic aneurysms (TAA).

An infrarenal aortic aneurysm with a diameter of 5.0-5.5 cm should be treated either with open surgical or endovascular therapy.

He underwent a CT scan which showed a 5 cm dilatation of his large artery in the abdomen called aorta (saccular infrarenal aortic aneurysm). This was treated by interventional radiology using pinhole techniques. A small access was placed in the right groin artery (right CFA) and repair was done endovascularly using a Medtronic Renu cuff. Introduction Endovascular aneurysm repair (EVAR) uses endografts, also called endovascular grafts, covered stent-grafts, or transluminally placed endovascular grafts to isolate an aortic aneurysm from the circulation.

The 3 patients with primary infected aneurysms required only hybrid treatment, whereas infected graft excision and new graft interposition was performed in 2 of the 3 patients with secondary infected aneurysms. Over 64% of infrarenal aortic aneurysms were treated with endovascular aneurysm repair (EVAR) in 2007. 1 Expanded indications have occurred as additional experience has been obtained using this technology and includes use of stent-grafts to treat aneurysms in the pararenal and visceral aorta, as well as emergent and nonaneurysmal pathologies. 2,3 This, however, has been associated with a rise Compared with open repair of infrarenal abdominal aortic aneurysms, endovascular aneurysm repair is less invasive and results in significantly better perioperative outcomes, including fewer systemic complications, shorter operative time, lower use of postoperative mechanical ventilation, and shorter hospital stay, resulting in a lower risk of perioperative mortality (7, 8). While aneurysms can occur along the entire length of the aorta, the infrarenal location is the most common.
